Work with individuals in the community. Shifts are based on the needs of the individuals and are usually from 6am-10pm. Hours may vary according to the day. We are seeking committed staff to provide the best support and care to individual’s in services. Enjoy outings and skills teaching, at the same time!
Home & Community Habilitation is a direct (face-to-face) service provided in home and in community settings to assist program participants in acquiring, maintaining and improving shelf-help, domestic, socialization and adaptive skills necessary to reside successfully in home and community based settings. Services consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, therapeutic activities, personal adjustment, relationship development, socialization and use of community resources. Through the provision of this service, program participants learn, maintain and/or improve skills through participation in a variety of everyday life activities necessary to live in the community, to live more independently and/or to be more productive and participatory in community life.
- Bachelor’s Degree in a human service field or;
- Licensed Practical Nurse (LPN) or;
- High School Diploma/GED equivalent plus a minimum of 2 years of developmental disability direct care experience or a minimum of 3 years related experience.
